If you are approved for a package, you will be placed in the Government’s national queue until your package can be allocated to you. There are options for accessing some services while you wait.
All applications for government-funded aged care services are made through My Aged Care (link opens in new window). Our Home Care Team are here to help you throughout every stage.

You will have an assessment by the Regional Assessment Service (RAS) or Aged Care Assessment Team (ACAT). You’ll discuss or show:

You are placed on a national waiting list for allocation of a Home Care Package.

You will also receive ongoing support and advice from your local WMQ Home Care Team. Your Care Coordinator is your key contact for anything related to your care. If your needs increase, we’ll talk to you about your options.

Wesley Mission Queensland Care experts will guide you through all the steps required to receive a home care package appropriate for your needs.
You will need to:
1, contact myagedcare.gov.au, the government agency for accessing all funded aged care services.
To register and provide information about your health and lifestyle.
2, Then the government's My Aged Care Assessment Team called ACAT will visit you at your home to assess which level care is needed. There are four levels of care, ranging from level one for basic support to level four for high care support.
3, after the assessment has been completed, you will receive an approval letter for a home care package.
You will then be placed on the national waiting list and it will take some time to be allocated your home care package.
While you wait, Wesley Mission Queensland can provide support or may be able to offer you temporary home care services if these are available, like the Commonwealth Home Support Program, government funding for entry level services or you can pay a fee for home care services from Wesley Mission Queensland until your package is allocated.

You have 56 days to choose a provider once you receive a letter confirming that you've been allocated a package. At this point, we'll work with you to develop a personalised plan.

It's a relief to know you've been assigned a package and now is the right time to contact us at Wesley Mission Queensland to help with next steps.
The home care package letter will confirm what level of care you've been assigned and provides a referral code which can be used to select a home care provider such as Wesley Mission Queensland.
Please keep the letter and the referral code as you will need that for the next part of the process.
You will have 56 days to select a provider, although you can receive an extension if required.
Once you have your letter, you can book a home care conversation with Wesley Mission Queensland and we can arrange to meet with you at your home to determine your specific needs and create a personalized home care program.
When we meet with you, we would like you to tell us about your specific needs.

What kind of services can you receive from a home care package?
There are four levels of services you can receive with a home care package from basic to complex home care needs.
Level one offers basic support.
Level two provides low level support.
Level three gives immediate support.
And level four delivers high care support.
The My Aged Care Assessment Team, also known as ACAT, will visit you to help determine what level package you can be approved for.
To give you an idea as to some of the home care services Wesley Mission Queensland can provide, if you are to receive a level two package, you could get personal visits by a dedicated Wesley Mission Queensland support worker to help you with cooking, shopping and transport to community activities.

What is a home care package and how is it funded?
A home care package is designed to assist you if you have more complex care needs that go beyond what the Commonwealth Home Support Program can provide.
There are four levels of government contribution available designed for people with different home care needs.
Level one provides support for people with basic care.
Level two supports people with low level care requirements.
Level three offers support for people requiring intermediate care.
And home care level four provides high care support with
Wesley Mission Queensland as your HCP provider.
You can expect a dedicated care leader who will coordinate all your care needs.
An individual care plan tailored to your needs.
Regular review of your plan to ensure it continues to meet your needs.
And support. And guidance to make the
most of your home care package funding.
There are no out-of-pocket costs for an HCP unless you are assessed as being required to pay a government income tested fee or you choose to purchase extra services. Wesley Mission Queensland has no hidden fees and offers the best value for your home care package.
There is no charge to set up services. No basic daily fee and no charge for our care workers to travel to you.
Detailed monthly statements are provided to show exactly how the government home care subsidy and any private contributions are being spent.
In addition, a tax invoice is provided that lists all your personal contributions owed and or made.
