Readiness check for the Logan Youth Foyer program

To be a participant in the program you must:

  • Be aged 16-25 yrs old. 
  • Be willing to live in a single tenancy, with no partners or children residing with you.
  • Be willing and ready to engage with program staff regularly and be willing to meet the ‘Foyer Deal’ across the 6 services we offer.
  • Be eligible for social housing through Queensland Government due to housing challenges you are facing. Program staff can assist you with applying.
  • Have an independent income (Wages, Centrelink or both).
  • Be ready to engage with education and/or employment.
  • Have the ability to manage a tenancy and the skills required to live alone with some limited support.

If you have any questions about this form, please contact us on (07) 3151 3830, Monday to Friday, 9am to 12pm. The purpose of the Readiness Check is to help us determine if you are ready for the Logan Youth Foyer Program and identify any goals you would like to work on. 

Important information:

  • This form should be completed by the young person aged 16-25 who is interested in the Foyer Program
  • If you need assistance from a support person you can ask them to sit with you
  • The information you submit will only be visible to the Logan Youth Foyer Team and a limited number of other WMQ staff who have a legitimate reason, and are authorised to, view your information.
  • Your information will be stored securely by WMQ

The Foyer is a community where young people have their own unit with shared communal spaces and a program of activities. The Foyer offers regular coaching and opportunities to young people. We will provide the space for you to create goals and aspirations for your life, and some examples of how we can help are below.

  • Study - complete school, a course or training opportunity
  • Employment - Get a job, or build on my career goals
  • Independence - build the skills I need to live by myself successfully
  • Community - make connections with others, make friends, have fun
  • Health and Wellbeing - improve my physical or mental health, work on particular health goals
  • Finances - get support with budgeting, save money, learn how to pay bills
  • Relationships - improve relationship with family or friends, work on boundaries, form and maintain positive relationships with others in my life
